People sometimes ask how a guy from the Bronx ends up in Minnesota.

It’s not a story I talk about publicly, and most people have never heard it.

When I was a resident at Columbia, I got a call from a headhunter about a practice in Minnesota. To be honest, I had zero interest in the Midwest. But they were persistent, and eventually I agreed to come out and meet the group.

The administrator picked me up at the airport. On the drive, he kept talking about how excited they were about a new surgeon they had just hired and how the two of us were going to be great partners going forward. I was excited too.

The next day, we were in a meeting when he got a phone call. The tone shifted immediately. He kept saying, “I’m so sorry. I’m so sorry.” Then all the color drained from his face. He stepped out of the room, came back in, and told me the surgeon they had just hired had been tragically killed the night before while trying to cross a highway.

It was shocking. There’s no other word for it.

I had flown in from New York, and the partners decided to still go ahead with dinner that night. It was one of the most awkward dinners I’ve ever been part of. No one really knew what to say.

At one point, the senior partner, a wonderful man who has since passed, looked at me and said, “Well, I guess we really need you now,” trying to break the tension.

From that moment on, something shifted. I fell in love with Minnesota. I fell in love with the people and the practice. I stayed.

It was the best decision I ever made.

I made the decision to become a physician because I wanted to fix broken people

The more I practice the more, I realize just how many people in our society are completely broken, whether physical, emotional or spiritual.

Clearly, dealing with people on a daily basis who have had very difficult lives, undergoing physical and emotional traumas can certainly be difficult and can lead to burnout

For me, however, the more I hear stories of those that are broken the better I want to become a trying to help them and it is something that continues to reignite the flame of why I went into this in the first place

Yesterday I interacted with A patient who was seriously broken both physically and emotionally. She pulled out of a folder, pictures of herself from years earlier and asked if she could ever become that person again. This was an intensely emotional moment and moments like these that become imprinted and they are the inspiration to try to do even more.

I see this every week.

Patients who have done everything they were told to do.

Doctor after doctor.

Labs that come back “normal.”

Medications that don’t change how they feel.

And yet their bodies keep breaking down.

Brain fog. 

Fatigue.

Sensitivity to environments others tolerate. 

A nervous system that never truly settles, always stuck in survival mode.

Most doctors don’t go into medicine because they want to rush people through appointments.

I was asked a great question after a speaking event yesterday.

How do you balance work and life when you are trying to do meaningful things?
My answer is simple, but it took me years to understand.

It is not balance.

It is peaks and valleys.

We all have a ceiling. 

A real breaking point.

That moment where we stop being sharp, stop being present, and start feeling scattered, disorganized, or disconnected. Everyone’s ceiling is different, and learning where yours is matters.

As you climb toward that peak, something interesting happens. 

Often nothing moves for months, and then suddenly, in a short window, everything changes. 

Decisions click. 

Opportunities align. 

That moment can redefine your career or your life.

But if you push past the peak, you fall off the cliff. That is burnout.
The valley is just as important as the climb.

That is where you step back.
That is where you recover.

That is where you reconnect with yourself, your family, and your purpose.
For me, that means time with my family, movement, meditation, reflection, and occasionally stepping away completely. 

Not because I am quitting, but because I want to come back clear, focused, and grounded.

Progress does not happen in a straight line.

It happens in cycles.

Know your peak.
Respect your valley.
That is how you stay in the game for the long run.

5 Personal Goals to to strive for in 2026

We are all fighting the same battle: The struggle to live a life of alignment, vitality, and deep, resonant purpose.

2026 isn’t about doing more but about being more.

Here are the 5 promises I am making to myself—and to you.

1. The Courage to Stand in Truth.

Authenticity has a cost and it will inevitably disrupt those who profit from the status quo.

For years, I measured my success by consensus of my peers. But I realized that if I am not upsetting the establishment, I am failing the people who need me most.

I am willing to be misunderstood by the establishment to be understood by the suffering.

I will speak the hard truths—exposing the gaps in our system and fighting for the patients others have left behind— When you stand in your conviction, you give others permission to stand in theirs.

2. Finding the Sacred in the Small.

We all wait for the "magic pill"—the big surgery, the breakthrough, the viral moment. But the miracle is found in the mundane. It is in the glass of water you drink with intention every morning. It is in the quiet moment of review when the clinic is empty. It is in the unglamorous, boring basics that no one sees.

Greatness isn't a flash of light; it is the discipline to show up for the small things, day after day, with the same reverence you bring to the big things. Extraordinary is doing the ordinary things EXTRA. 

3. Going to the Final Mile.

Most of the world stops at "Good Enogh” Get the patient "stable" and move on. But the final 5%—the distance between surviving and thriving—is where the magic lives.

I strive to be the person who falls in love with that final mile. The one who pushes for vitality, not just existence. Unreasonable devotion to the final mile is the purest form of love.

4. The Responsibility of Radiance.

We are taught that self-sacrifice is a virtue. We burn ourselves out to serve our organizations and our patients, believing that our exhaustion is proof of our love.

We are wrong. You cannot give what you do not have. I practice every longevity hack, bio-optimization strategy, and meditation protocol that I teach my patients—not out of vanity, but out of duty. My body is the instrument through which I serve. If I am not vibrating at my highest frequency—if my mind isn't sharp, if my energy isn't clean—I cannot show up fully for you or my family.

5. The Power of Surrender.

I am a surgeon. I love control. I love plans, data, and schedules. But I have learned that you cannot schedule a miracle.

Some of the most profound breakthroughs in my clinic didn't come from a protocol; they came from a moment of unplanned connection. I strive to leave space for the detour. To trust that when the map doesn't match the territory, the territory is trying to teach me something.

I’m frustrated, and I think patients deserve honesty about why.

My job as a spine surgeon is to help people heal. 
To help people who have been living with pain for months, sometimes years. 

People who have done physical therapy, injections, medications, and everything asked of them, only to be told they still have to live like this.

And then, when a solution is recommended based on evidence, experience, and what is best for the patient, insurance companies deny it.

I’m seeing more and more denials for total disc replacement, often for reasons that simply don’t make sense.

Patients are denied because they have degeneration at other levels, even though preserving motion at the painful level reduces stress on the surrounding spine.

They’re denied because they have leg pain, even though removing the damaged disc and replacing it addresses that exact problem.

They’re denied because they’ve had prior decompression surgeries, despite data showing these patients can do just as well when they meet the right criteria.

Instead, patients are often denied a motion-preserving option that their surgeon believes is appropriate, leaving fewer choices on the table and delaying care for people who are already in significant pain.

Here’s the part that’s hard to accept.

People pay their insurance premiums every month. 

They pay deductibles. 

They follow the rules. 

And when they finally need the care they’ve been paying for, the answer is no.

Why are patients paying for coverage if medically appropriate care is denied when it’s needed?

This isn’t about overuse.
This isn’t about unnecessary surgery.
This is about patients being blocked from options that may give them better function, less pain, and a better quality of life.

We’re going to keep advocating. 

We’re going to keep pushing back. 

And we’re going to keep standing up for patients who deserve access to the care they’ve paid for.

You can’t force a body to heal.

Spending New Year’s in Cabo with my wife and my boys was exactly what I needed. 

Family time matters more than anything, and most weeks I’m moving nonstop between the ER and the clinic, fully present for my patients from morning to night. 

That work is important, but so is stepping away long enough to be fully present with the people who matter most to me.

Getting away creates space.

 Space to think, to reflect, and to reconnect with why we do what we do. 

When things finally quiet down, it’s incredible how many ideas surface and how much clarity comes back. We don’t always realize how loud life gets until it isn’t.

I’m coming back refreshed and recharged, batteries fully reset. 

This week alone I’ll see close to 100 patients between my two clinics, and I’m genuinely excited to get back to work. 

Every patient deserves full attention, thoughtful care, and a plan that actually moves them forward.

These moments with family are what allow me to show up better for my patients, just like being present with my patients all week reminds me why being present with my family matters just as much.

Looking forward to seeing everyone starting tomorrow and helping as many people as possible get back on the road to recovery.

In a  conversation with Dr. Louis Saeger, we talked about something that’s easy to forget about the human body.

You may look the same as you did 20 years ago, but at a cellular level, nothing is the same. Your body is constantly being torn down and rebuilt.

People ask me all the time what my favorite supplement is when I need to stay sharp and perform at a high level.
