The four red flags I see in nuclear medicine referrals every week:

🚩 Vague clinical question 

→ "Rule out mets" or "assess disease burden" - These are too broad to guide a focused report

My response: "What's the specific decision point? Are we deciding between surgery vs systemic therapy? Nodal staging that changes surgical approach? Progression vs post-treatment change?"

When the question is clear, I can tailor the protocol and focus my report on what actually matters.

🚩 Wrong test for the biology 

→ FDG PET for well-differentiated thyroid cancer (wrong tracer) 

→ Bone scan when PSMA PET is available and would answer the question better 
My response: "Is this the right test for this clinical question and tumor biology"

🚩 Wrong timing for disease tempo 

→ Repeat scan at 3 months when disease is slow-growing and 6-month intervals are standard - We're imaging too frequently for the tempo of disease

My response: "Does the scan timing match the treatment tempo?"

🚩 Result won't change management 

→ "PET to see how treatment is going" - but you're continuing same therapy regardless 

→ "Bone scan for staging" - but patient already has visceral mets needing systemic therapy anyway

My response: "If positive, does management change? If negative, does management change?"
If both answers are "no" - we're documenting, not deciding.

I'm not gatekeeping to say no.
I'm asking these questions because a well-framed referral leads to a more useful report.

Across 15+ years working as a counselor and therapist, I’ve noticed that the standard approach to anxiety is teaching people to manage it.
   
And of course, there are all kinds of tips and tricks for managing anxiety.  

A quick google search will pull up over 300+ million results. 

Don’t get me wrong, I get it. Anxiety sucks bad! So, if there’s a quick fix you take what you can get. 

The problem is managing begets more managing.  

There is no endpoint to all of the stressors and triggers you encounter over your lifetime. There is no finish line where you can stop and relax.  

Over time, managing anxiety can turn into a seemingly never-ending hamster wheel where you feel both exhausted and too afraid to stop because that feeling might catch up to you and overwhelm you.  

Unfortunately, many of the clients I work with come to me after years and years of therapy, continuing to “manage” the same symptoms that they started with. They feel defeated because what they’ve been told to do isn’t working. 

But that’s only one approach to addressing anxiety. 

It is not your only option. 

I share a different approach with my clients and the most common response I hear is, “Thank you Mr. Green. I feel like I have hope again!”

When someone experiences real change, it’s like you can feel something shift in them right in front of you. You can see the wheels turning in their head, the excitement in their voices, and the new energy coming off of them.  

Experiencing that alongside my clients is a huge reason why I do the work I do. There's nothing like helping someone achieve a new revelation where they can clearly articulate new solutions and approaches to the problems they thought were too overwhelming for them. 

So now I'm on a mission to introduce a new approach to help people become immune to anxiety instead of just managing it. 

Here's how we will do it.

➛ Develop a new perspective on what's happening to and in you. (Making sense of standard human equipment, the mind and body)
➛ Create a new relationship with your emotions. (Skills for emotional agility)
➛ Creating and experiencing a deeper sense of core identity. (The foundation you stand on in the face of uncertainty. This is where you replace fear—anxiety with trust—inner knowingness.)
