Patients with complex chronic illness wait an average of 9 to 13 years to receive a diagnosis. For some patients, the delay can be as long as 20 years.

Two years ago, I had an idea to compile a directory to help patients save time while looking for clinics, providers, trials, patient communities, and educational resources.

Today that platform has grown to include a biomarker and treatment database for complex chronic illnesses like Long COVID, ME/CFS, POTS, MCAS, fibromyalgia, and related conditions.

The Spooniverse is for every patient who was ever told their labs were normal and summarily dismissed afterwards.

These patient communities have been ignored for decades. The least we can do is make sure the information that exists is accessible.

At the 2025 IACFS/ME Research and Clinical Conference, I presented the Pathogen Effector Convergence Theory (PECT), the idea that taxonomically diverse pathogens have independently evolved functionally convergent effector proteins that disrupt a shared set of cellular regulatory pathways, producing the overlapping clinical phenotypes of ME/CFS, Long COVID, PTLDS, and other post-infectious syndromes.
 
My theory makes five claims. First, effector proteins of taxonomically distant pathogens show functional convergence despite minimal sequence or structural homology.

Second, the pathways targeted by this convergence constitute a predictable set of critical regulatory hubs governing energy metabolism, immune activation, and stress adaptation.

Third, disruption of these hubs creates self-sustaining pathological cycles through epigenetic reprogramming, inflammasome activation, metabolic rewiring, and tissue remodeling—mechanisms that perpetuate illness even after the initiating pathogen has been cleared.

Fourth, this architecture explains the characteristic treatment resistance of many post-infectious conditions, as blocking any single upstream receptor or pathway node fails because convergent effectors and redundant activation maintain the disrupted state (image below).

Fifth, clinically distinct disease “subtypes” may sometimes represent temporal progression within the same underlying process of pathway dysregulation, and this mischaracterization has direct consequences in regards to treatment selection.
