Seven years ago today, our dear daughter, Leslie Sharon Kough, aka "Lulu," ascended into heaven. 

She was 18 years old.  Suicide.



I have written about Lulu on this platform many times. 

Some of you have been with me since the beginning... 

Some of you are reading about her for the first time. 

To all of you, thank you for being here. 

Your love has carried our family in ways you may never fully understand. 



Before you read any further, know this:  this is not a post about death. 

This is a post about love. 

Lulu's love. 

And the love she continues to pour into all of us.



Seven is a number that means something to me. 

In our faith, seven represents completion. Perfection. The fullness of God's plan. 

I do not pretend to understand that plan. 

But I have spent seven years learning to trust it. 

Seven years of learning that grief does not define me. 

That loss does not own me. 

That the deepest pain I have ever endured has also produced the deepest love I have ever known.



Lulu was the greatest servant leader I have ever known. 

I do not write those words lightly. 

The most profound example of leadership I have ever witnessed came from a teenage girl who poured herself into her relationships with developmentally disabled classmates… 

Who volunteered at the VA hospital on Sundays, listening to veterans' stories, calling each one "sir," and thanking them for their service… 

Who knelt beside the homeless at the shower trucks with the Order of Malta, serving with a heart so pure it could only have been a gift from God. 



That was Lulu. 

Pure love in action.



Seven years later, Lulu's fingerprints are still everywhere. 

I see them in her siblings.

Servant leaders as a firefighter and paramedic, an Apache pilot and Company Commander, and as an Army surgeon.

I see them in strangers who reach out through this platform to tell me that something I wrote about her gave them the courage to ask for help. 

I see them in every one of you who has sent a message, said a prayer, or simply held space for our family. 



You have loved us through the unimaginable. 

That love matters...  heals...



I don't believe in coincidences. 

I believe in blessings. 

And Lulu continues to bless us every day, even now, from heaven.



To those of you who are suffering right now… please hear me.

You can still find joy. 

I found mine through prayer, and when I finally raised my hand and my heart and asked for help. 

That decision changed my life.

Beavis and Butt-Head Understand Medtech’s AI Strategy Better Than Medtech Does.

I’m helping surgeons launch AI integration companies and I’m around Med Tech leaders sniffing around AI. 

It’s a scene from this Saturday Night Live skit. 

Senators asking questions that made you cringe.

Executives answering with words that meant nothing.

And somewhere in the background, Beavis and Butt-Head going “heh heh… he said algorithm.”

I kept thinking: this is medtech’s AI moment.

Fictional hearing. Real energy.

SENATOR: “Can you explain how your AI platform improves surgical outcomes?”

MEDTECH EXEC: “Absolutely. Our proprietary deep learning infrastructure leverages real-time intraoperative data to create a connected ecosystem of intelligence across the care continuum.”

SENATOR: “So… does it work?”

MEDTECH EXEC: “It’s a force multiplier.”

Beavis: “Heh heh. He said multiplier.”
Butt-Head: “Yeah yeah. He has no idea what it does.”

Here’s what I know after working with surgeons for two decades.

The ones winning right now are not waiting for their medtech partner to figure out AI strategy.

They are building their own.
Their voice. Their data. Their reputation. Their distribution.

The buzzwords are somebody else’s problem.

The outcomes are theirs.

For years in medtech, I lived inside a trade show booth.

Not figuratively. Literally. You showed up early, you stood tall, you waited. The philosophy was simple: be there, stay visible, and when a surgeon walks by, you go get them. Chase them down the aisle if you have to. That was the job.

I understood it. I did it. And a part of me always felt like there had to be a better way.

When I left medtech and started working directly with surgeons, I made myself a promise. I would never ask a surgeon to come find me. I would never ask for an appointment. I would never show up in their world with my hand out.

Instead, I started sending hyper-personalized videos. Seven of them. To seven different surgeons I genuinely wanted to help. Before 7:01 AM. Every single day. No pitch. No ask. Just value, delivered with intention, before most people have had their first cup of coffee.

That ritual changed everything.

So fifteen years later, when I do show up at a conference, I am not spending $40,000 on carpet and a back wall. I am not wandering the floor hoping to run into someone. I already know exactly who I am there to see. 

I fly into Nashville or New Orleans or LA, I stay in the shadows, I rent a bougie Air BnB, nearby, I put my culinary school to work, feed a surgeon, and I have one conversation that actually matters.

Then I get on a plane and go home.

I spent a decade waiting for surgeons to walk past a booth.

Now they ask me what I’m cooking next trade show.

My father was 65 when he died.

He had run marathons. He was one of the top tennis players and golfers in the South. 

Death. Nobody saw it coming. 

That’s the thing about unexpected loss. It doesn’t negotiate. It doesn’t give you time to prepare a response.

What it does give you, if you’re paying attention, is a moment of absolute clarity about what matters.

My mother stood in the middle of that grief and did something I’ve thought about almost every day since. She prayed the Prayer of Jabez over my life.

If you don’t know it, here’s the short version. Jabez shows up in 1 Chronicles almost as a footnote. Two verses in the middle of a genealogy that most people skip. He asks God for four things. Bless me. Enlarge my territory. Let your hand be with me. Keep me from harm.

That’s it. No elaborate theology. No conditions. Just a man asking to matter more than his circumstances suggested he would.

My mother prayed that prayer over me the way only a woman who has just buried her husband can pray something. With everything she had left.

And I made a decision.

Not just about my career or my company or what I was going to build. I made a decision about her.

She needed to mother me. I knew that. Grief does that. It reaches for the familiar things, the relationships that still make sense when everything else has stopped making sense. But I also knew something else. She needed to feel needed. Not just loved. Needed.

So I hired her.

She controls everything that touches money. Bookkeeping. Accounts payable. Accounts receivable. Hiring. Making sure everyone gets paid. She is the financial spine of everything we’ve built.

The agency doesn’t run without her. That’s not sentiment. That’s just true.

People talk about building a business like it’s a solitary act. Like the founder is out there alone, making something from nothing through sheer force of will. 

But every company I’ve ever respected has someone in it who holds the thing together quietly. Someone whose name isn’t on the press release but whose fingerprints are on every single decision that actually kept the lights on.

That’s my mom.

She prayed that my territory would be enlarged. And then she showed up and helped me build it.

I don’t know what promise you made in your hardest moment. I don’t know who was standing there when the call came or what you decided in the days after.

But I know this. The Prayer of Jabez isn’t a prosperity prayer. It’s a purposeful expansion prayer. It’s the prayer of someone who believes their life is supposed to count for more than it currently does and is willing to ask, out loud, for the help to make that happen.

My mother believed that about me before I believed it about myself.

The least I could do was build something worthy of the prayer.

The female surgeons I work with are among the most clinically rigorous people I've encountered in this space.

They are also operating inside a structural trap that their male counterparts never think about once.

Here's what behavioral psychology has documented - not opinion, peer-reviewed research - about how expert women are evaluated: women in high-status professional roles are assessed on two axes simultaneously. 

Competence and warmth. Men in identical roles are effectively assessed on one.

This asymmetry has a direct consequence that nobody in healthcare thought leadership is naming clearly enough.

Ask Kiran V. Patel, MD - we spend an entire dinner discussing such topics. 

Content that optimizes for warmth - relatable posts, personality-forward content, appearance-based visibility - suppresses perceived competence in the specific audiences where authority matters most. Peers. Industry. Referring surgeons. The medtech VP deciding who gets the advisory board call.

The female surgeon doesn't lose followers. She loses legibility in the rooms where likability is irrelevant.

This is not a critique of any individual surgeon's choices. It is a diagnosis of a market asymmetry that was not designed by surgeons and was not designed to serve them.

The surgeons I work with who understand this stop asking how to appear more relatable. They start asking what signal the right audience is actually calibrating for. That question leads somewhere completely different - to clinical specificity, to intellectual point of view, to a platform the authority economy can actually act on.

The research on this goes back decades. Heilman (2001). Fiske et al. (2002). The mechanism is documented. The application to digital reputation is newer — and it is the thing nobody is building a communication narrative framework around yet.

If you are a female surgeon building a platform and you have ever felt the gap between your follower count and the quality of your inbound opportunities -this is worth understanding. 

The gap is not a content problem. 
It is a signal problem. 
And it has a structural explanation.

When Chad Smith played a Keith Moon song on the same kit, hitting the same notes, you could still hear Chad in every single strike. 

That's not imitation; it's an identity so distinct it shines through someone else's material.

Every surgeon understands this journey.

Fellowship is years spent playing another's song. You learned to expose the spine exactly as your attending did, to sequence the procedure just as you were shown. You suppressed the instinct whispering, "I would do this differently," because your task was not to innovate, but to replicate.

The finest surgeons, however, eventually earn the right to deviate. They introduce a modified approach, a preference in sequencing—a subtle shift that becomes their signature. They stop being a cover band and become artists in their own right.

Yet, almost none were taught to articulate this evolution. The world sees a generic surgeon, not the master who spent a decade becoming impossible to replicate.

This is where my work begins. I work as a surgeon reputation architect - to my knowledge for more surgeons than anyone I know.  

Not to make surgeons louder, but to make them identifiable. So that even when they perform a familiar procedure, their unique signature is unmistakable in every movement.

How I spend my Saturdays as an empty nester Surgeon Reputation Architect (my full batch day routine):

1. I don't wake up with a hangover. Sounds obvious. It isn't. Thank goodness for Athletic Brewing. Non-alcoholic beer changed my Saturdays more than any productivity system.

2. I'm up at 6 AM. Dressed in something I wouldn't be embarrassed to be seen in. That detail matters more than you think.

3. I walk into my studio and spend the first block of the morning creating 40 personalized videos. Thirty seconds. Sixty seconds. Ninety at most. Each one is for a surgeon I want to help that week. Every week for five years, I've chosen seven new surgeons. Seven. Not a hundred cold DMs. Seven people I've actually researched, thought about, and decided deserve a real introduction. That discipline is the system.

4. Then I go outside. Move around. Get some air. 

5. I come back and spend 90 minutes on my existing clients. I'm a surgeon reputation architect. That means I'm tracking orthopedic, spine, and interventional pain trends so my surgeons don't have to. I'm thinking about who they should be connecting with — a media decision maker, a medtech partner, a podcast they should be on. That 90 minutes is where the real strategic work happens.

6. Then I leave the house again. I shop for what Sweet Sandi and I are cooking that night. Six nights a week, we cook. A culinary school education turned into a Whole Foods membership is one of the best decisions I've ever made.

7. At 2 PM, I sit back down for exactly 90 minutes. In that window, every one of my LinkedIn posts for the week gets finished. Not drafted. Finished.

By Saturday evening, my week is already built. Monday through Friday, I'm not creating. I'm executing and delivering.

The empty nester thing is real. The house is quieter. But the tradeoff is that Saturdays belong entirely to the work. No one needs a ride. No one needs dinner at 6. That space is the asset.

Most people think they have a productivity problem. They don't. They have a structure problem. Saturday is the structure that makes every other day possible.

Most people post when the algorithm says to. I’m about to find out if that’s the wrong variable entirely.

Starting this weekend, I’m running a public experiment. The hypothesis: your ideal audience’s availability matters more than platform-recommended timing. 

For me, that means posting almost exclusively on weekends — two to three posts Saturday, two to three on Sunday — and going nearly silent during the week.

My audience is surgeons. Surgeons are in the OR Monday through Friday. They’re not scrolling LinkedIn at 9 AM Tuesday because they’re harvesting a graft. They’re reading on Saturday morning with a coffee, when the pager isn’t going off and they have something rare: mental space.

The conventional playbook says post Tuesday through Thursday, 7 to 10 AM, two to four times a week with 24-hour spacing. That playbook is built on aggregate data. It has nothing to do with who specifically you’re trying to reach and when they’re actually present and receptive.

There’s a second variable I’m testing: weekend continuity. Each Saturday and Sunday becomes a focused series - two to three posts building on a single theme. Not scattered content. Sequential thinking. So the surgeon who reads Saturday morning catches Part 1, comes back Sunday, and finds Part 2 waiting.

The algorithm may penalize me for frequency. 

The question is whether depth of audience resonance outperforms breadth of algorithmic distribution.

One question worth sitting with before you optimize your next post: are you timing your content for your calendar - or for theirs?

Now for the part that’s going to sting.

Old-school medtech leaders are watching reps like her and reaching for words like “off-brand” and “compliance risk.” What they won’t say out loud is what it actually is: proof that the model they built their careers defending is losing ground.

-The model where the company owns the relationship.
-The model where the rep is the accessory, not the asset.

That model is being quietly dismantled by people with a camera and a consistent point of view.

The medtech dinosaur doesn’t have a following. She does.

That’s not a platform problem. That’s a leverage problem.

And leverage has always determined who gets to write their own terms.

Ted Williams divided the strike zone into 77 cells.

Each one roughly the size of a baseball. He calculated his batting average for every single cell. Low outside corner? .230. Belt-high, middle-in? .400+.

His philosophy came down to one discipline. Only swing at pitches in your happy zone. 

Even when the umpire called it a strike/when the crowd bood/when the manager wanted more aggression.

He refused.

.344 lifetime average. Highest on-base percentage in baseball history. The last human being to hit .400.

Williams did not have the best swing. He had the best discipline. He understood something most people never figure out.

You do not win by doing more. You win by knowing where you are .400 and refusing to swing anywhere else.

I have observed two surgeons who remind me of Williams more than they would ever give themselves credit for.

These two surgeons will elbow me in the rib cage for drawing attention to them. 

Dr. Charlie DeCook built 12x12. Twelve total joints by lunch. Not by moving faster. By mapping every second of the OR the way Williams mapped every cell of the strike zone. 

Every role defined. 
Every instrument anticipated. 
Every wasted motion eliminated. 

After filming him: 

It is not speed. 
It is precision that looks like speed because nothing is wasted.

The Trust Trigger Most Surgeons Miss

Personalized video doesn't change the quality of the signal; it changes the physics. For the first time, what happens inside the room can travel outside it—intact, unmediated, in your voice, and with your reasoning. The gap between your clinical reality and what the market understands closes, not because you marketed better, but because you made the invisible visible.

This is why surgeons often misunderstand personalized video. They see a dentist use it and think: engagement tactic, retention tool, nice touch. They miss the deeper mechanism.

Personalized video isn't just a relationship gesture. It's a demonstration of your cognitive fingerprint.

When someone watches a sixty-second video of a surgeon thinking out loud—explaining their reasoning, what they weighed, and the choices they made—something happens that no credential can produce. The viewer doesn't just learn about the surgeon; they get a felt sense of the surgeon's mind. This felt sense is the foundation of every durable reputation.

Most surgeons are described by others. Ratings platforms use stars. Institutions use volumes. None of these descriptions capture what truly differentiates a top-tier surgeon: the specific texture of their clinical thinking.

Personalized video is the first format that makes this texture transmissible.

And transmission is what scale requires. Reputation doesn't scale through content volume; it scales through depth of impression. A surgeon with a hundred generic LinkedIn posts creates a hundred opportunities to be forgotten. A surgeon who sends one sixty-second video that makes a viewer feel they’ve encountered a unique mind has created something that gets forwarded, referenced, and remembered—not because it was well-produced, but because it was specific enough to feel true.

Specificity is the mechanism. The trust trigger isn't video; it's precision. Video is just the only format with enough resolution to carry it.

A written post can tell someone how you think. Video shows them. In a market where reputations are built on what others have written, the surgeon who shows their mind directly isn't just more visible.

They're in a different category entirely.

That’s the reputational architecture. The question isn't whether personalized video works. It's whether a surgeon is willing to be that legible—to let the market see not just their achievements, but how they actually think.

Most won't. That's why it works for those who do.

No lame hotel rooms. No suits. No keynote stage.

Just a room full of physicians and healthcare leaders who were tired of being visible without being valuable.

The post with 4,000 views produced no new referrals.

The post with 300 views produced three conversations that led to two advisory relationships.

Most surgeons never audit this. They look at the number LinkedIn shows them — views, likes, reactions — and optimize for making it bigger. The platform rewards this optimization because bigger numbers benefit the platform.

The numbers that matter to a surgeon building an authority practice are not on the dashboard.

The affinity economy and the authority economy do not share metrics. 

Engagement rate measures how well content resonated with a general audience. It tells you nothing about whether your authority in a specific domain increased, whether your name moved up the mental stack of the referring physician who was already considering you, whether the medtech scout who landed on your profile understood what you're the expert on.

These are invisible metrics. They compound quietly. They do not appear in a weekly analytics report.

The surgeon who has spent eighteen months optimizing for engagement may have a larger audience than they started with and less clarity of position — because the content choices that maximize engagement are precisely the choices that dilute positional authority. Relatable posts. Popular opinions. 

Agreeable observations. None of them stake a claim.

A smaller number of people knowing exactly what you are is worth more than a larger number of people knowing your name.

Stop asking how many people saw it. Start asking whether the right people read something that changed how they think about who to call.

I spent years in MedTech as a VP of Sales deciding which surgeons got the call.

I know exactly how the KOL designation works. Who gets selected. Why they get selected. And what the company is actually buying when they put a surgeon’s name on a roster.

They are not buying your surgical excellence. They never were.

They are borrowing your credibility to move their product. Those two things have been dressed up as a partnership for three decades and I was inside the machine long enough to know nobody talks about that distinction out loud.

The surgeon produces the authority. The company borrows it.

I left MedTech to build something on the other side of that equation. And the first thing I noticed when I started working with surgeons on their own reputation architecture is how many of them were auditing themselves against criteria that were never designed with their interests at the center.

Waiting to be designated. Waiting to be placed on a panel. Waiting for an external party to publicly confirm what their patients and colleagues already knew privately.

I watched surgeons with fifteen years of category-defining outcomes sit invisible online while the industry circled the same rotating faces at the same conferences.

That is not a talent problem. That is a system problem.

And the surgeons who understand the system stop playing by its rules.

Here is what I have seen happen when a surgeon stops treating the KOL designation as the destination and starts treating their own reputation as the asset.

And I’m doing just fine.

Surgeons are the only profession in the world that spent a decade earning the most trusted credential in society, and then decided the key to an audience was pretending they didn't.

Edward Everett spoke for two hours at Gettysburg.

Nobody remembers a word he said.

Lincoln stood up and spoke for two minutes.
272 words that rewired a nation.

The problem is almost never competence.

Surgeons are trained to be thorough. That training becomes a liability the moment you confuse thorough with comprehensive. Your patient is not a resident. They are scared. They have been Googling for three weeks. They have a spouse in the waiting room who is also scared. They do not need the biomechanics lecture.

They need four things from you before that surgery.

1. What we are doing. 
2. Why we are doing it. 
3. What recovery looks like. 
4. What could go wrong, in language a nervous person can hold onto.

That's it. That's Lincoln's framework applied to the OR.

The surgeons I work with who have the highest patient satisfaction scores are not the ones who explain the most. They are the ones who communicate with the most precision. They have done the hard work of editing themselves down to what actually matters in that room.

Clarity is not dumbing it down.
Clarity is respecting the moment enough to show up prepared for it.

Everett was brilliant. He just never figured out that a shorter sentence can carry more weight than a longer one.

Your patients will not remember everything you say before surgery.
Make sure they remember the right things.

Most surgeons building a LinkedIn presence are optimizing for the wrong audience.

Not because they're doing it wrong. Because nobody told them there are two entirely different evaluations happening simultaneously — and most platforms are built to pass only one of them.

The patient-facing evaluation is obvious. Accessible language. Relatable content. Evidence of care and approachability. Surgeons understand this instinctively, and most of what gets built is built here.

The industry evaluation is different. A VP of Medical Affairs who lands on a surgeon's profile is running a different algorithm. They're not looking for warmth. They're looking for evidence that this surgeon can hold clinical complexity in one hand and commercial strategy in the other — and translate between them in a room full of people who don't speak surgery.

What fails that evaluation isn't bad content. It's content built entirely for the wrong room.

The signal a medtech partner needs isn't follower count. It isn't engagement. It's pattern recognition across 90 days of content — does this surgeon have a specific clinical point of view, can they connect outcomes to business implications, and does their platform reflect intellectual rigor rather than general popularity?

Most don't. Not because they lack the clinical thinking. Because the platform was architected for a different outcome.

This is the misread. 

Surgeons assume that building an audience is building authority. But audience and authority are not the same asset. A surgeon with 12,000 followers and appearance-forward content has built something real. It's just not what gets the advisory call. It's not what gets the consulting agreement. It's not what gets you into rooms where the clinical voice shapes product strategy.

Visibility was never the problem. Platform architecture was.

The question I have for surgeons: If a VP of Medical Affairs reviewed the last 90 days of your content today, what would they conclude about your clinical thinking — and would they make the call?
