Nobody told me the best ideas would come this late.

I turn 62 today.

Not a milestone post. Not a birthday celebration. More of a field report.

Because here’s what I didn’t expect.

The sharpest thinking of my life is happening right now. Not at 32 when I commuted to work by parachuting from perfectly good airplanes. Not at 42 when I thought I had all the answers as a Med Tech VP. Not at 52 when I started to realize I didn’t.

Here’s what I’d tell the 32-year-old version of me:

	1.	Figure out how SMALL is big enough. Everyone chases scale. The real money is in obsession with a niche no one else wants to own.
	2.	Find your where. The what and the how will come trailing along. Stop planning the whole trip before you pick the destination. Would you rather be the Prince of San Diego or the King of the Ozarks?
	3.	The difference between an employee and an entrepreneur is $1. One dollar of your own revenue changes your brain chemistry forever.
	4.	Figure out if you’re running towards something or away from something. Both feel like motion. Only one gets you somewhere.
	5.	Get busy building your own dream life. Because if you don’t, you’ll spend your best years building someone else’s.
      6. Nobody is coming to pick you. Pick yourself. Then build something so good they wish they had picked you first.
	7.	Your story is the asset. Not your resume. Not your title. Not your credentials. The thing only you lived through is the thing no one can compete with.

The truth? The person with 30 years of pattern recognition, battle scars, and a niche they’re obsessed with is the most dangerous person in any room.

You’re not running out of time.
You’re just now seeing the whole map.

I hired my daughter.

I got a front row seat to watch Sierra become something I never coached her to be. A creative. A real one. The kind who sees angles in a story before you’ve even finished telling it. The kind who makes content that doesn’t sound like content.

But that is the reality for my beautiful cousin. Early onset Alzheimer’s. The kind that arrives decades before anyone expects it and forces a family to rewrite every plan they ever made.

I’m learning / this disease is a thief. It doesn’t announce itself nor negotiate. It just starts taking. Memories first. Then moments. Then the person you thought you had more time with.

Most people don’t think about it until it walks into their family.

It walked into mine.

My word for 2026 is authorship.
Not the kind you think.

Yes, I co-authored a bestselling book.
Yes, we are launching another one this year.
But authorship taught me something I was not expecting.

The best things I have ever built were co-authored.

2025 best-selling book, men@pause: Life 2.0, was co-authored by two buddies I grew up with. 

Physician BrandRx was co-authored with Scott Sigman MD who believed reputation could be systematized.

Content Crib was co-launched six years ago with my buddy Eric I Anderson for healthcare leaders who wanted to learn how to become thought leaders. 

The Orthopreneurs Symposium was co-launched with Ryan “Saw-Bones” Molli, D.O. & Chris McClellan, DO  to equip surgeons who refused to separate their clinical identity from their entrepreneurial one.

Ortho Mafia Poets was co-launched w/Dr. Scott Sigman as a community that understood the best surgeon thought leadership does not sound like marketing. It sounds like conviction.

Required a refresh. 

We canceled all of our big medtech business. Every contract. Every retainer. Every safe, predictable revenue stream.

Instead we went all in on 100 surgeons.

Quietly in 2025 becoming reputation architects. 

Unconventionally, we started with professional athletes. Learning how to help them turn “pro” in something other than their sport. 

Next, we began working with orthopedic, spine, and neurosurgeons. Studied what trust looks like when the stakes are a career, a body, a legacy. Already learned that reputation is not what you say about yourself. It is what the right people repeat when you are not in the room.

We approached med tech leaders, thinking they would be interested in thought leadership. Only 1. Others were indifferent. 

We had a plan. Going all-in. We became the surgeon reputation architect. A job description you will not find. 

Not a marketing agency. Not a social media manager. Not a content mill with a healthcare vertical.

An architect. Someone who studies the blueprint before building. Someone who understands that a surgeon’s reputation has a structure, a foundation that has to hold before you ever add a story.

Now we are expanding. Interventional pain. Cardiac.

Not because we are chasing growth. Because surgeons in those specialties started asking why no one was doing for them what we were doing for ortho and spine.

We opted out of launching campaigns for medtech giants. We opted in to brand launches for medtech startups and surgeon-founder ventures.

Your reputation is being written whether you author it or not.

LinkedIn just got a lot smarter. And most surgeon content just got a lot more invisible.


The platform rolled out a new algorithm that reads your posts semantically. That means it is no longer counting keywords. It is reading for meaning. It is assessing whether you actually have a coherent point of view or whether you are just producing content that sounds like every other surgeon on the platform.

Here is what that means for you practically.

If your last 30 posts could have been written by any surgeon in your specialty, the algorithm now knows that. It cannot distinguish you from the noise. So it does not distribute you.

If your posts, read as a body of work, tell a story about who you are, what you believe, and what you stand for clinically, the algorithm begins to associate your profile with that topic. 

Distribution follows authority.

This is the KOL to DOL transition playing out at the algorithmic level.

LinkedIn is now rewarding surgeons who have built a point of view, not just a posting schedule.

Three things to do right now.

1. Stop posting tips. Start posting beliefs.

2. Pick three to five ideas you return to repeatedly and approach them from different angles, different patient stories, different clinical moments.

3. Own specific language. The surgeons who become the algorithm’s default recommendation for their niche are the ones who consistently occupy the same semantic territory.

Clinical excellence got you to the table.
Narrative capacity is what keeps you at it.

If you insist on writing the way you have always written, talking about the same things your peers have always talked about, refusing to learn the frameworks of narrative transportation, perspective, and problem solving, and objecting to the visual structure of how content actually gets read on this platform, the algorithm is not going to argue with you. It is just going to ignore you.

The algorithm just made that official.

Hospitals spend hundreds of thousands dollars on marketing departments that make every surgeon look identical. That's not a branding strategy. That's a camouflage strategy.

Professional headshot. White coat. Arms crossed and a tagline about "compassionate, patient-centered care."

Now multiply that across every surgeon on the hospital website.
Same photo style. Same language. Same everything.

That's not a branding strategy. That's a camouflage strategy.

The surgeon who replaced 5,000 joints and pioneered an outpatient protocol gets the same bio template as the surgeon who graduated residency last year.
The surgeon who developed a technique that changed how an entire procedure is performed gets the same "Dr. Smith is passionate about helping patients return to active lifestyles" copy as everyone else on the page.

The hospital didn't build their brand. The hospital buried it.
Not intentionally. But effectively.

Because hospital marketing departments aren't designed to differentiate individual surgeons. They're designed to protect institutional consistency. 

Those are two completely different objectives.

So the surgeon with 20 years of expertise, a philosophy that changes how patients think about their own surgery, and a track record that medtech companies would pay to be associated with...

...looks identical to every other name on a dropdown menu.

That's not a visibility problem. That's a translation problem.

The clinical excellence exists. The reputation architecture doesn't.

And until that changes, the surgeon's expertise compounds inside the hospital while their reputation flatlines outside of it.

Surgeons don't need to leave their institution to fix this. They need to stop relying on it as their only megaphone.

Your hospital markets the system. You need to digitize trust.
And those are two very different prescriptions.

9 Ways We’re Using AI to Engineer Surgeon Reputations in 2026

We built an entire reputation engine.

Here’s how we’re using AI across our surgeon reputation architecture practice right now:

	1.	Competitive practice intelligence. We use Claude and Perplexity to research and map every competing practice in a surgeon’s local market. Digital presence, content output, patient review profiles, referral patterns. Our surgeons never walk into a meeting uninformed again.

	2.	DRG and CPT coding analysis. We utilize MedScout to pull procedure volume trends, reimbursement patterns, and coding data to show surgeons where the real revenue opportunities live in their market. This is business intelligence most surgeons have never seen packaged this way.

	3.	Psychological voice profiling. We run every surgeon through Humantic.ai to capture their authentic communication style, DISC profile, and decision-making patterns. No more guessing how a surgeon sounds. We know before we write the first word.

	4.	Audience and gatekeeper profiling. We profile the people our surgeons need to reach. Medtech executives, hospital administrators, conference curators, TV producers. Then we tailor every piece of outreach to match how that person processes information and makes decisions.

	5.	TEDx and speaking stage acquisition. We use Claude to map every TEDx event and conference keynote opportunity within range of each surgeon. We research the organizers, identify topic white space, and craft applications calibrated to each curator’s psychology using Humantic.

	6.	Primetime media outbound. When a health story breaks, we have 24 to 48 hours to get our surgeons in front of producers. We monitor news cycles with AI, build media-ready one-sheets for each surgeon, and pitch directly to producers with outreach customized to their booking preferences.

	7.	Referral network mapping. We research the referring physician landscape around each surgeon’s practice to identify relationship gaps and untapped referral sources. MedScout provides us with referral data - specific to #referrals. Then we build content strategies that fill those gaps.

	8.	Visual presentation design. We use Gamma AI to turn strategy and research into investor-grade pitch decks, keynote presentations, patient education materials, and LinkedIn carousels that look like a top consulting firm designed them.

    9. Sales conversation intelligence. Before a surgeon walks into a medtech partnership meeting or hospital negotiation, we run the prospect through psychological profiling so our surgeon knows exactly how to lead the conversation. I talk to surgeons every week who have a course idea, a podcast concept, a consulting offer, or a product vision sitting in their head. They've had it for months. Sometimes years. It hasn't launched because they're trying to do it alone between cases and call nights.

I also talk to healthcare entrepreneurs, medtech founders, and digital health builders who have the playbook, the platform, or the distribution but no trusted clinical voice attached to what they're building.

Meaningful work can quietly train you to believe that availability is the same thing as faithfulness.

It isn’t.

For a long time, I measured my usefulness by how quickly I responded, how much I carried, how often I stepped in when others were unsteady. I told myself this was love. I told myself this was commitment.

What I didn’t see was how easily service can become substitution.

Instead of staying rooted in who I was called to be, I kept drifting toward whatever crisis needed me most. Over time, that does something subtle to your soul. You stop standing in truth and start orbiting demand.

When you consistently meet people in motion, they don’t encounter clarity.
They encounter access.

And access is easily mistaken for value.

Eventually, I learned that steadiness communicates more than effort ever could. Not withdrawing. Not disappearing. Just remaining where you’re meant to be, even when others want you to move.

That shift costs something.

Some relationships thin out.
Some roles fall away.
Some expectations dissolve.

At first, it feels like loss.

Later, you realize it was sorting.

What remains are relationships that don’t require exhaustion to survive. Connections rooted in presence, not rescue. Mutuality instead of dependence.

The work doesn’t stop being meaningful.
It just stops demanding everything.

If you’re in a season where your world feels quieter, more spacious, and slightly uncomfortable, this might not be regression.

It might be formation.

Faithfulness often looks less dramatic than we expect.
But it endures longer.
