The four red flags I see in nuclear medicine referrals every week:

🚩 Vague clinical question 

→ "Rule out mets" or "assess disease burden" - These are too broad to guide a focused report

My response: "What's the specific decision point? Are we deciding between surgery vs systemic therapy? Nodal staging that changes surgical approach? Progression vs post-treatment change?"

When the question is clear, I can tailor the protocol and focus my report on what actually matters.

🚩 Wrong test for the biology 

→ FDG PET for well-differentiated thyroid cancer (wrong tracer) 

→ Bone scan when PSMA PET is available and would answer the question better 
My response: "Is this the right test for this clinical question and tumor biology"

🚩 Wrong timing for disease tempo 

→ Repeat scan at 3 months when disease is slow-growing and 6-month intervals are standard - We're imaging too frequently for the tempo of disease

My response: "Does the scan timing match the treatment tempo?"

🚩 Result won't change management 

→ "PET to see how treatment is going" - but you're continuing same therapy regardless 

→ "Bone scan for staging" - but patient already has visceral mets needing systemic therapy anyway

My response: "If positive, does management change? If negative, does management change?"
If both answers are "no" - we're documenting, not deciding.

I'm not gatekeeping to say no.
I'm asking these questions because a well-framed referral leads to a more useful report.
