Lead MDS coordinator for 100+ bed skilled nursing facility. I provided oversight, education, and in-servicing as needed to other MDS personnel under my supervision. Assisted with the facility’s case management of skilled insurance residents by coordinating interdisciplinary plan of care, completion and tracking of physician certification as well as collecting and submitting documentation for review. Case management duties also included coordination of benefits such as verify and collecting co-payments. 
Briefly worked in a part time consultant role, conducting on-site analysis of MDS assessment and Medicare processes (including system analysis) while identifying provider opportunities for improvement. Writing reports to summarize audit and consultant review findings to include corrective action plan recommendations, as needed.

Duties include Monitor/investigate timely and accurate billing and recording (Provider Liable, AAA), provide on- site support for Myers and Stauffer exception reviews and other state case mix audits, Monitor financial outcomes and set opportunities for achievement, 5 star QM analysis, triple check management and redevelopment, Responsible for conducting and organizing Continuing Education, in-service, meetings, Survey and Regulatory updates, Promote facility program development,, Documentation reviews to support services, Manage denials process, and monthly operations report.

As Interim Director of Nursing, I was responsible for the recruitment and development of nursing staff and ensuring that all nursing personnel follow policies and procedures.
Other duties in this role included planning, directing, coordinating and evaluating nursing care services. Ensuring that the facility is in strict compliance with all state and federal guidelines, which includes an active role in the preparation for all state surveys, and taking all necessary steps to create a plan of correction.  I maintained a liaison with residents, families and support departments to plan for resident needs.

Employed as an MDS coordinator/ Clinical Manager for 55 bed facility.  Responsible for implementing and coordinating managed care products, as well as identifying and developing plan of care in accordance with Medicare Skilling criteria, care planning, PPS assessments, and MCD case mix optimization to improve facility reimbursement.  Under my tenure as MDS coordinator the facility’s MCD reimbursement was increased 5%.
Management and oversight of the Quality Measure reporting.  Assisted administrator with the quarterly PB&J submissions.  Responsible for monthly infection control tracking, as well as scheduling and coordinating Resident Care Conferences. Completed quarterly case mix audits and reimbursement optimization of 2 additional buildings in the Deaconess Long term care company. 
In my role as clinical manager I contributed to and pesented monthly and PRN in- services for the nursing staff. Verified staff attendance at in- service and maintained in- service logs for each nursing team member.  Assisted the Director of Nursing with STNA scheduling and daily staffing assignments. 
Coordinated the facility’s Performance Improvement program by reviewing monthly audits and synthesizing the data into reports to review with the QI committee quarterly. I also performed monthly audits of the medication pass and medication carts to ensure staff adherence to state and federal guidelines.  I completed daily nursing note review to ensure accurate, concise, and timely charting by the staff nurses.  Assisted and supported the DON and acted on her behalf during her absence, and  supported the overall facility goals, policy and chain of command.
