Monitoring every stage of client care life cycle with regularly implementing and updating customer touch points within ABS brightstar care SAAS.

Recognised for testing system wide changes in report running and technical support for the branch as first port of call. Apart of the initial team in establishing the Harlow Reablement contract and led the application referral project in outlining the referral pathway and deploying the new referral process centred on Discharge Home to Assess model to Commissioners, APT and MDT

Performing server administration tasks, including security permissions, user and group admin, resource monitoring, identifying any event logs and warnings and live call monitoring, incident and rapid response to critical alerts and triaging appropriately using CACI and ESR(Electronic Staff Records)

Maintaining internal infrastructure requirements on site and across all different locations within ECL including testing, maintaining and providing training for new staff hardware. I routinely interact with help desk and other teams to assist with troubleshooting and identifying root causes and providing technical support.

Running daily database reports, staff utilisation reports  using internal software to ensure functionality of system and access data for daily meetings and to provide Reablement  training and awareness workshops and forums.

Supervising and monitoring the running of the reablement care service with high level reource planning, scheduling and route planning.  Documenting data related to patient care in care plans, assisting social care placement for long term domiciliary care. Ensuring all information is up to date on care database and server, routinely updating network server information related to staff and clients.  This coordination included liaising with community services including social care and Patient discharge teams to support the specific patient discharge pathway with recognised individual care needs.

Ensuring the daily monitoring of staff absence, sickness, holiday and creating staff rostering to meet business KPIs.

Superuser and Care Resource Planner

Identifying potential growth and communicating with stakeholders to actively promote services provided and assist customers to stay in their own homes 

Participating in comprehensive clinical discussions regarding management of an assigned group of clients, collaborating as member of the Multidisciplinary team in assessing and planning care according to service users’ health needs and eligibility into the service. Facilitating safe and appropriate discharge from acute inpatient care back to the community 

Coordinating crisis and rapid response referrals to commence in our reablement service within two hours of patient being discharged home or referral being received using Coldharbour  - Civica care management software 
 
Routinely provided on-call support and covering staff shortages in office and the field.

Acute Hospital Patient Care Facilitator 

Intake team lead in referral pathway process involving processing of new referrals and care packages as per suitability of service. Assigning case-holders to service users in an appropriate manner using effective care staff planning and rostering using healthcare specific ERP software

Interviewing new candidates and tracking applicants through the pipeline

Handling internal and external complaints through CIAMS and appropriate chain of command and data protection.

Maintaining high standards of customer care, ensuring effective communication and understanding any
individual and cultural care needs of that of the customer.

Participating in clinical discussion of service users with Ward Teams, Care Coordinators, Hospital at home teams and Support and Recovery workers.

Following good practices of data protection with handling with sensitive patient Information with access to Electronic medical records. Recommendation usage of trend micro and secure email platform via outlook servers in receiving and sending referrals and information about relevant client databases

Training and coordinated intake team with delegation of tasks and managing the running of the service from point of referrals, accepting declining care packages, scheduling and rostering care packages to commencing the service within 24 hours and in some cases same day start.

Regular review monitoring of staff and clients to ensure continuity as well as feedback. Completing live call mniotirng to ensure all care calls and initial assessments and ereivmwes were proper assigned and completed and devices were utilised correctly log time and travel time. 

Overseeing coronation of staff across all four regions of company managing company sickness, holiday request, requesting additional bank staff and ensuring support is covered in the field. 

Successfully assisted in creation of training manuals and recommendations for efficient usage of data monitoring and referral tracking.

Plus, I'm eager to specialise in the incredible world of AI and GPT multimodal models as well as continuing to dive deeper into the latest Security Superbadge and its prerequisites. 

In my downtime, you'll find me soaking up the magic of theatrical productions and listening to scores I've also discovered a new passion for mechanical keyboards in which I recently built a lego keyboard. I also manage my Ubiquiti managed cloud controller network, adding Synology NAS, learning Linux through hypervisor and raspberry PI, AP placement,  IoT configuration and VPN.
