Through the Transition Advantage solution, we worked with hospitals, physician groups and payors to enhance their care transition process by coordinating the care team, helping patients adhere to care plans and identifying gaps in care through technology and human touch. 

My responsibilities included creating alliances with strategic partners like National Rural Health Association and hospital group purchasing organizations.  I worked to negotiate, contract and build relationships with the largest group purchasing organizations (GPOs) in the country, including MedAssets (now Vizient) and Amerinet (now Intalere). Once the contracts were in place, I would build training programs and educational events for GPO staff and their hospital clients.
