I thought I had early dementia.

I couldn't remember things, lost focus mid‑task, and struggled to understand what I was doing. I kept it to myself — too afraid to say anything at work or home.

Later, I discovered the truth: it was perimenopause brain fog.

A temporary condition — but one that had shaken my confidence and disrupted my business. 
No one had warned me. 

And I now know I wasn't alone.

A 2022 study by the National Library of Medicine found 90% of women hadn't been taught about menopause.

Why this matters for care leaders
In adult social Care:

80% of the workforce is female

The average age is 44, the exact time perimenopause symptoms occur.

Brain fog, anxiety, fatigue, low mood, and hot flushes are common symptoms.

Left unaddressed, menopause symptoms are often mistaken for:

Lack of commitment

Legal and leadership expectations

Under the Equality Act 2010, menopause can be classed as a disability if symptoms significantly affect day-to-day functioning.
And now, the upcoming Employment Rights Bill (2026–2027) will introduce a mandatory Menopause Action Plan for large UK employers.

From 2027, companies with 250+ employees must publish their support approach, including:

Menopause policy and guidance

Manager training and awareness

Reasonable workplace adjustments (ventilation, uniform changes, appointment flexibility)

Culture-building to reduce stigma

Monitoring and improvement cycles

Most organisations have wellbeing policies.
Far fewer have leaders who can comfortably talk about issues that impact wellbeing.


A few years ago, I was doing some voluntary work 
I had a conversation with a pensioner who, among other things, 
told me he was a widower and attended his church daily.

When I saw him again a month later,
 He recalled the entire conversation.

This surprised me,
 So I asked some of my older friends why this was the case

 They said I was probably the only person he had had a proper conversation with since we last spoke!

It reminded me how easily we underestimate the power of conversation.

A single exchange that demonstrates genuine interest can reduce someone's sense of isolation and help them feel valued.

The same principle applies at work.

Conversations are one of the most underused strategic tools in workforce culture.

Dialogue can:
 
✅Challenge assumptions that prevent progress on wellbeing and inclusion.

✅Reframe leadership understanding of menopause, mental health, and psychological safety as workforce priorities.

✅Turn wellbeing from policy intent into workplace practice.

As workplace strategists, we should not underestimate the cultural impact of a single, well-framed conversation, especially the awkward ones.

The conversations organisations tend to avoid are often the ones that build trust and shift behaviour.

Try these conversation strategies

1. Lead with curiosity, rather than correction.
"What's making this difficult?" instead of "Why isn't this done?"

2. Wellbeing is delivered by line managers, not policies
Menopause and mental health become real for staff through line manager responses —not organisational statements.

3. Treat discomfort as a signal, not a barrier.
Culture-change conversations feel uncomfortable because they challenge norms. Avoiding them preserves the status quo.

Look for the indicators that prove a policy has actually left the paper and entered the culture.
If not, you have policy documentation but no transformation.

Care Sector Workforce Culture & Retention Strategist | Menopause & Mental Health Risk | CQC Well-Led Systems

The biggest change in employment law in three decades is coming.
 It is time to learn from the past to protect your organisation's future.


I love learning about law, so I took the opportunity to attend two events discussing the numerous upcoming changes to employment law.

We were told it represents the biggest shift in employment legislation in around 30 years. 

For managers in the care sector, staff retention and workplace culture will be even more important!

One of the many tribunal cases discussed involved an employee who had disclosed menopause symptoms and experienced declining performance due to brain fog.

She had been a good performer for approximately four years.

But she was placed on a performance improvement plan and given a rating that resulted in no pay rise.

However:

• No referral was made to occupational health
 • No reasonable adjustments were put in place
 • The impact of menopause symptoms on her performance was not properly considered

She eventually resigned and brought a claim to the employment tribunal.

They held that her menopause symptoms amounted to a disability because they were long-term and had a substantial adverse effect on normal day-to-day activities, including concentration and memory.

She was awarded £65,000 for disability discrimination.

Cases may not end up in tribunals due to bad intentions 
but rather in the case of menopause, it  is due to  a lack of system

 That does not support staff members and therefore will not protect the care organisation.n


How can you prevent your organisation from ending up in this situation?

Well, a policy that will help:

✅ Staff feel safe to disclose concerns about symptoms
✅tell you when you should treat menopause  as a potential disability
✅Make reasonable adjustments so that you can perform the role well despite symptoms

"Why does she get off early while we’re still here?”



I recently read about an employee who was allowed to leave early to complete her write-ups from home as a reasonable workplace adjustment.

The adjustment itself was not the problem.
The reaction to it, however, became a litmus test for the culture.

The above statement was made and
She was bullied

In a mistrustful culture, a workplace adjustment is interpreted as:
– Special treatment
– Extra workload for others

Leaders aim for fairness in their workplaces

But when adjustments are treated as burdens rather than opportunities to:
– fulfil legal duties
– improve retention
– strengthen trust
and gossip about colleagues is tolerated, the culture is revealed

Failing to protect an employee from gossip or hostile commentary about adjustments can amount to disability-related harassment under the Equality Act 2010.

Under UK GDPR, health information and workplace adjustments should only be shared on a strict need-to-know basis.

Managers hold two responsibilities at the same time:
– They protect their team members’ confidentiality and dignity.
– They acknowledge team workload pressures without blaming the individual who has the adjustment.

When a manager fails to do this , they inadvertently teach their other team members that

Their Disclosure may not be safe.
Support may trigger resentment.
 Their Privacy may not be respected.

So psychological safety reduces.

Adjustments do not damage culture.
It exposes the weaknesses that are in it

One way that may help is

The A D J U S T S framework is a system for confidential and fair workplace adjustments:

A — Access route
Have a standard, confidential process for staff to request adjustments (manager/HR, documented template).

D — Define confidentiality
Explicit “need-to-know” boundaries; health reason separated from operational rota info.

J — Joint agreement
Written agreement on what (if anything) is shared with colleagues and by whom.

U — Understand & communicate.
Managers explain adjustments without oversharing; set legal responsibility, etc., and set boundaries/zero tolerance on gossip, as this can be seen as disability harassment.

S — Support workload balance
Adjustments paired with conversation on impact, staff/workload review, so the team can raise concerns without attacking individuals, and duties are rotated so they don’t land on one person.

T — Train leaders
Practical training on Equality Act duty, handling personal health, and how to handle “Why does she get X?” conversations.

S — Scrutinise & review
Track adjustments made, review trust in the adjustment process, survey outcomes; review regularly.

3  Myths care providers often believe about menopause compliance 
that CQC may explore under Well-Led 


Myth 1: “We have a wellbeing policy so that menopause will be covered."

Will it?

If a staff member's menopause symptoms begin affecting performance, what would your organisation’s process trigger?

Support or capability?

According to UK Government data*, 43% of employers have no menopause policy or support measures at all.

Having a policy is a starting point.

But CQC is not only interested in whether a policy exists.

Under the Well-Led domain, inspectors often look for evidence of:
• Manager awareness
 • Reasonable adjustments
 • Supervision conversations
 • Staff confidence to raise health concerns

A policy that lives in a folder nobody reads is not evidence of implementation.

It is just paper!

Myth 2: "Nobody has raised it, so we are probably fine."

Silence does not always mean absence.


I spent years masking symptoms I thought were early dementia — and said nothing.

I was not alone!

Research from the House of Commons Women and Equalities Committee found that less than a third of women tell anyone at work they are going through menopause.**

Silence can reflect:

• Reduced confidence
 • Fear of career impact
 • Low psychological safety


This is why CQC may examine patterns such as:
• absence data
 • turnover trends
 • exit interview themes
Those patterns often tell a deeper story about leadership and workforce culture.

Myth 3: "Menopause is a personal health issue, not a business issue."
In law, menopause symptoms can meet the definition of disability under the Equality Act 2010 when they are long‑term and substantially affect day‑to‑day activities.​

That makes menopause not only a health issue, but a workplace responsibility.
I recently heard a care manager say he had a young team.

So, no need for menopause training.

I wondered if he knew how young menopause can start.
And if older members were to join, what would be the best time to address it?
From April 2026, statutory sick pay will be payable from day one of absence.
For a sector already managing rota pressure and retention challenges, proactive menopause support becomes a workforce strategy, not simply a wellbeing initiative.​

What helps organisations demonstrate Well‑Led leadership is:
Visible menopause policies
Confident manager conversations
Reasonable adjustments
Cultures where staff feel safe speaking up

Is your company culture like a family portrait at your friend's house?


It looks happy from the outside.
But the lived experience inside can feel very different.

My understanding of culture began long before my professional life
Watching my widowed mum lead our family taught me lessons no management book ever could.

She carried multiple responsibilities.
Ran her own business.
Parented alone.
Had minimal outside support.

But what she understood instinctively:

When resources are limited,
 Leadership matters more.

We, the children, all had responsibilities.
Not randomly assigned.

Delegated by age and capability.
And yes, sometimes gender.

The pressure did not disappear.
She redistributed it.

When circumstances are less than ideal, culture is built on three things:

1️⃣ Clear Communication of Reality

She didn't pretend things were easy.
Leaders who acknowledge pressure build trust and manage expectations.

2️⃣ Delegate by Readiness, Not Fairness

The oldest did more.
The youngest did what they could.
I cooked, and I was good at it.

Not equal but effective.

In teams, burnout happens when responsibility and capability are misaligned.
In adult social care, issues like menopause and mental health can temporarily affect capability.

If unacknowledged, that misalignment compounds retention risk.

3️⃣ Shared Contribution

We knew our effort reduced the work for our mum.
Even when we didn't enjoy doing housework, we understood we had a duty to her.

When staff see how their work makes a difference, commitment strengthens.

When effort is unrecognised, people either:

 keep going out of principle
or quietly withdraw from extending  themselves

In care, this is more than a low morale issue.

When staffing is tight,
agency costs are rising,
and CQC expects Well-Led cultures,

Leaders cannot remove pressure.
But they can redistribute it shrewdly.
.
A family portrait does not fix dysfunction.
Just like a mission statement doesn't create culture.

But strong, supported leaders can.

 In high-pressure environments, leadership becomes the safety net.
And your culture determines whether staff can cope or prefer to leave.

3 Myths Care providers may believe

I self-diagnosed with dementia.


I was struggling to:

Remember
 Express myself clearly
 Process information

If you were my manager, would your system support me, or would it manage me out?

I masked the symptoms.
 and intentionally reduced interactions 
to reduce being discovered

Now imagine that experience affecting 80% of your workforce.

In adult social care, it can.

 As the workforce is over 80 per cent female and has an average age of 44, which puts them in the perimenopause chapter of their life

My symptoms were caused by menopause brain fog.

Many care professionals suffer in silence, concerned they will be judged as"not fit for the role".

Is that contributing to high turnover, where experienced staff leave one provider for another rather than leaving the sector entirely?

 Menopause is not a well-being topic.
It is a workforce strategy issue.

The shift from discretionary support to a legal requirement is approaching quickly.

From 2027, the Employment Rights Bill will require large UK employers to publish a Menopause Action Plan, alongside wider employment reforms that will affect providers of all sizes.

The risk: providers intend to care but lack the infrastructure to implement.
The consequence: avoidable churn of experienced staff, friction under the Well Led domain, and unnecessary CQC scrutiny.

I am partnering with one care provider to pilot a structured Menopause Implementation Framework.

This is not awareness training.

It is an operational audit and structured delivery session focused on:

• Systems of adjustment — confidential frameworks that work in a 24-hour care environment
 • Leadership capability — moving managers from discomfort to competence in complex conversations
 • Commercial evidence — capturing data to demonstrate impact on retention and culture
